Ontario Fallen Fire Fighters Memorial
On Sunday, October 6, 2024, Ontario’s fallen firefighters will be honoured for their bravery, heroism, and sacrifice at a special ceremony held at the Ontario Fallen Fire Fighter Memorial located on the southeast corner of Queens Park. The service will begin at 1:00 PM and is expected to last approximately one hour. The deadline to submit names is July 31, 2024.

Gone But Not Forgotten
For the first time since the pandemic started, Ontario’s fallen firefighters were honoured with an in-person memorial on Sunday, October 2, 2022. The families of the fallen impacted by the cancellations in 2020 and 2021 were invited to join in person for the ceremony. The names of 36 fallen firefighters who made the ultimate sacrifice in service to their communities were added to the Memorial Wall, in addition to the 98 names of those lost in 2020 and 2021.

Please fill out the form below to submit a name to the Ontario Fallen Fire Fighters Memorial in Toronto for the 2024 Ceremony.
Make sure that in addition to filling out this form that you also send a copy of the WSIB approval letter to admin@ontariofirefighters.org or your submission will not be accepted. Please note that the WSIB letter must be a PDF and signed by the adjudicator. Once your form and letter are received, they will be reviewed by the committee. If the submission is accepted, the family will receive an invitation in late summer to the Memorial at the address that you enter on the form below. |
Ontario Memorial Decorating Policy:
The Ontario Memorial Committee understands the importance of memorializing lost loved ones. We understand the significance of leaving pictures and personal items on the walls to recognize and remember those that have fallen in the line of duty.
If family members choose to leave a decoration or photo up on the memorial walls following the ceremony, we support you in doing so. In some circumstances, a photo or memento can be large enough to block another person's name on the walls, so please be respectful of all fallen fire fighters names. Please be advised that the OPP Office at Queen's Park will remove and dispose of any items left for longer than a week after the ceremony. |
